Marla McCants lost more than 500 pounds after My 600-lb Life — and her transformation today is nothing short of extraordinary

Marla McCants remains one of the most unforgettable participants ever featured on My 600-lb Life. Once weighing more than 800 pounds and unable to stand or walk, she lived in constant fear for her life. Desperate for help, she joined the show — and what happened afterward would change everything. Today, she has shed over 500 pounds, and her transformation is remarkable.

Maintaining a healthy lifestyle isn’t easy for anyone. Between family responsibilities, limited time, and the expense of wholesome food, it’s easy to fall into the trap of fast, unhealthy choices. Marla was no exception. Bedridden and battling a severe food addiction, she spent her days eating from her bed and even kept a small fryer within arm’s reach.

When Marla first met Dr. Younan Nowzaradan — known to fans as Dr. Now — he wasn’t sure he could help her. Over the years, he has turned away patients who refused to follow medical guidance, insisting that he can only help those willing to help themselves. Still, Marla relocated to Houston with her daughter and underwent gastric bypass surgery in hopes of saving her life.

But recovery wasn’t smooth. Marla struggled with depression, cravings, and a deep reluctance to stand or walk. Eventually, Dr. Now discharged her, unsure whether she would continue the fight. Her daughter Sierra, unwilling to give up, begged the doctor for one more chance.

Over time, Marla began making progress. In a follow-up episode, she had dropped to 534 pounds but still struggled to walk. Her cravings persisted, and she admitted to secretly eating foods she knew she shouldn’t. Yet she also expressed hope — she wanted to be present for her grandchildren and dreamed of one day having her excess skin removed.

Behind the scenes, Marla later claimed that the production exaggerated certain moments for drama, including scenes filmed shortly after surgery when she was malnourished and physically unable to stand. Despite the hardships, she continued pushing forward on her own terms.

And the results have been incredible.

By 2019, during an appearance on Sister Circle, Marla revealed that she had lost a staggering 580 pounds. She spoke candidly about the loneliness she felt at her heaviest and her determination never to return to that painful place again. Today, Marla advocates for compassion and understanding for those battling food addiction — something she calls a lifelong struggle.

Her message is simple: never judge what you don’t understand, and never underestimate the power of persistence. If she could reclaim her life, she believes others can, too.
