The Real Housewives of New Jersey star Margaret Josephs has revealed the results of her latest cosmetic procedure, sharing that she recently underwent a nose job as part of a broader beauty makeover.

The 58-year-old reality TV personality debuted her updated look on Instagram this week, posting photos that documented both the procedure and her recovery. Josephs explained that the “nasal rejuvenation” was done to address changes she noticed as she got older.

According to Josephs, the tip of her nose had started to droop whenever she smiled, something that increasingly bothered her in photographs. She noted that she never disliked her nose when she was younger, but over time the cartilage no longer supported the tip the way it once did, which also made a small bump on her nose more noticeable.

Eventually, she decided to consult Beverly Hills plastic surgeon Dr. Gary Motykie for what she described as a subtle adjustment. After the procedure, Josephs said she feels her nose now suits her face perfectly, adding that the change is noticeable but natural enough that people can’t quite pinpoint what’s different.

Experts say this type of procedure is becoming increasingly common among older patients. Beverly Hills surgeon Dr. Deepak Dugar previously explained that noses and ears can continue to grow and droop with age due to gravity and cartilage changes. As a result, many people in their 40s, 50s, and beyond begin to notice their noses appearing longer or more downward-pointing compared with earlier photos.

Dugar, who is known for his “scarless nose job” technique, said he has seen a significant rise in rhinoplasty patients over 50 in recent years. While this age group used to represent a smaller portion of his clientele, it has now become the second-largest group seeking the procedure after patients between 18 and 25.

He added that many older patients today are more familiar with anti-aging treatments thanks to the popularity of facelifts and other facial rejuvenation procedures. However, because a facelift doesn’t address the nose, many people eventually look to rhinoplasty to refine the center of the face and create a more youthful balance.

Josephs didn’t stop at the nose procedure. She also underwent a fat transfer and stem cell treatment to restore volume to her face, further enhancing the overall rejuvenation.

The reality star has already been showing off her refreshed appearance at public events, including BravoCon last November, and during a recent outing to a Broadway show in New York City.

Josephs has been open about her history with cosmetic procedures. In 2019, she underwent extensive facial surgery, including a revision neck lift, deep-plane facelift, and both upper and lower eyelid lifts performed by plastic surgeon Dr. Mark Karolak. The following year she also had a breast lift and reduction by Dr. Joseph Michaels.

She has previously spoken candidly about maintaining her look through treatments like Botox and occasional laser procedures, noting that she believes surgery can sometimes achieve results that topical products cannot.

Meanwhile, Josephs is rumored to be returning for a future season of The Real Housewives of New Jersey, which was paused in 2024 as producers worked on retooling the long-running Bravo series.
